Multiplate®ProstaglandinE1(PGE1)isaresearchreagentkitfor theassessmentofADPtestHSand assessmentofpositivecontrolsoftheADPtest ontheMultiplate®instrument. Fortheresearchevaluationofqualitativeplateletdefectsaswellas plateletfunctioninhibition.PGE1enhancesthesensitivity ofADPtesttoplateletfunctioninhibitionespeciallytothe effectofclopidogrelinresearchstudies.Moreover,theadditionofahigher doseofPGE1intothesamplecaninduceanabnormal aggregationinADPtest,whichallowstheperformanceof anpositivecontrol.ProstaglandinE1(PGE1)isusedincombinationwiththe ADPtestreagent(MP0120).IntheADPtestHS,anincompleteblockadeofaggregationisoftenseenwithclopidogrelstudies.ThismaybeduetothefactthatADPnotonlytriggerstheP2Y12 receptorontheplateletsurface(e.g.,thereceptorthatisblockedbyclopidogrel),butalsootherADPreceptors(whicharenotaffectedbycopidogrel).AcombinationofADPandphysiologicalplateletinhibitor(prostaglandinE1(PGE1))canbemoresensitiveforthedetectionoftheactionofclopidogrelthantheuseofADPalone.ThebindingofADPtotheP2Y12receptorreducesthelevelofcAMPintheplatelet,whichinturnenhancesthereleaseofcalciumfromendogenoussources.Theenhancementofintracellularcalciumleadstotheactivationandaggregationoftheplatelet.PGE1reducesthemobilizationofcalciumandthusinhibitsplateletaggregation.AsclopidogrelalsoreducestheplateletactivationbyADP,clopidogrelandPGE1actsynergistically.

MeasurementPrinciple:

PGE1isanaturalplateletinhibitorwhichtriggersan increaseincAMPlevelsintheplatelet.cAMPisasocalled secondmessenger,i.e.anintracellularsignalling molecule.AdecreaseofthecAMPlevelintheplatelet leadstoplateletactivation.AnincreaseofthecAMPlevel counteractsplateletactivation. Theadditionof20µlPGE1totheADPtest(9.4nMPGE1 finalconc.)inducesamoderateinhibitionofplatelet activationinhealthynormalbloodsamples,buta significantincreaseofsensitivityoftheADPtesttothe plateletinhibitionbyclopidogrel. Theadditionof50µlPGE1intoADPtest(22nMPGE1 finalconc.)normallyinducesastronginhibitionofADP inducedaggregation(positivecontrolforADPtest).
