Background:
HeparinisawidelyusedantithromboticagentthatcanbeadmiNISTeredviaanintravenousorsubcutaneousroute.Itisnaturallyoccurringandtypicallyderived from bovineorporcinetissue. Theanticoagulanteffect ofthissulfatedglycosaminoglycanresultsfromitsbindingtoantithrombin(AT),themajorinhibitorofthecoagulationcascade.Bindinginducesaconformationalchange intheAT molecule,whichgreatlyacceleratestheAT inhibitionofseveralcoagulation proteases.TheacceleratedinhibitionofthrombinandFactorXabyheparinistheprimarybasisforitsanticoagulationproperties.
Twoformsofheparinareusedtherapeutically:unfractionatedheparin(UFH),withanaveragemolecularweight(MW)of15,000andlowmolecularweightheparin(LMWH),withaMWbetween4,000and6,500.ThelowerMWresultsinLMWHactingprimarilyonFactorXa,whereasUFHefficientlyinhibitsboththrombinandFactorXa. Bothheparinformsshouldbemonitored.UFHcanbemeasured withanaPTTorananti-XachromogenicassaywhileLMWHshouldbemeasuredusingandanti-Xachromogenicassay.NotethatthiscontrolisforresearchuseonlyintheUSandCanada.
