Background:
ThevonWillebrandFactor(vWF)isalarge,multifunctionalglycoprotein,occupyingakey positioninprimaryhemostasis.Ithasamultiplestructurewithseveralfunctions:
- ItisthecarrierproteinforFactorVIIIinplasma;itformsacomplex andthusprotectsFactorVIIIfromearlyproteolyticdecomposition.
- Itactsasamediatorforplateletaggregationbyattachingitselftoplateletmembrane receptors(GPIbandGPlIb/IIIa)followingpreviousplateletactivation.
- Itplaysapartinprimaryhemostasisbyactingasamediatorbetweenadhesionedplatelets andthesubendothelium(lesionedvascularwall).
Inordertoanalyzetheadhesiveproperties,asaruletheplateletaggregationismeasured (measuringsystem=ristocetin-dependentplateletaggregation). However,thisdoesnotreflectthephysiologicalsettingnorthefunctionofthevWF.For determiningtheadhesivepropertiesofthevWF,itsbindingcapacitytocollagenservesasa parameterwhichcorrespondstothephysiologicalfunctionofthevWF.
